Un PDF describe cómo hay que pintar una página. Por dentro no contiene títulos, párrafos ni celdas de tabla: contiene instrucciones para colocar glifos en unas coordenadas. Esa asimetría explica por qué escribir un PDF es fácil y volver a leerlo no lo es, y por qué cualquier conversor que prometa una ida y vuelta perfecta está exagerando.
Estas herramientas están construidas en torno a lo que sí se puede recuperar. Las imágenes de página salen exactas, porque renderizar es lo único para lo que un PDF está diseñado. El texto sale de forma fiable. La estructura —tablas, columnas, estilos— sale tan bien como permita la maquetación interna del archivo original: excelente en un PDF exportado desde Word y tosca en uno producido por un escáner.

Why is my converted Word file not identical?
Because the structure was discarded when the PDF was made. The converter rebuilds headings, paragraphs and tables by reading positions on the page, which works well on a cleanly exported document and less well on a complex layout. Expect to fix a few things rather than nothing.
What about scanned PDFs?
A scan is a picture of a page, so there is no text inside it to extract and these tools will hand back an empty document. Run it through OCR first — that recognises the characters and adds a real text layer, after which converting behaves normally.
Which format should I pick?
Pick by what you intend to do next. Editing means Word. Numbers mean Excel. Putting a page into a slide or a message means JPG or PNG. If you only need the words and nothing else, plain text is the format least likely to arrive mangled.
